Collingsworth Family Medicine is housed in a new 6,891 sq. ft. facility that opened in January 2010 as part of a multi-million dollar hospital expansion project. It has been equipped with state-of-the-art equipment and designed with the patient in mind. The clinic is staffed with medical providers who are devoted to keeping the whole family healthy.

Chronic Care Management

The Chronic Care Management Program at Collingsworth Family Medicine was established to help Medicare patients with diabetes, arthritis, high blood pressure, heart disease, stroke and other chronic conditions to play a more active and effective role in the management of their health.

Wendy Lankford, LVN, a certified Clinic Health Coach, meets with each patient enrolled in the program to talk about their health care needs, concerns and goals.  Together, Langford and the patient will develop an individualized plan to accomplish these goals, and will monitor progress over time, making adjustments to the plan as needed.

If you have a chronic condition, and would like to join the health improvement team, call 806-447-5311 to schedule an Annual Wellness Visit with your primary care provider to discuss enrollment. Wendy cal also be reached at the same number to answer any other questions you may have about the program.

Coaching services provided by Wendy are offered free-of-charge to eligible Medicare patients.

 

Medicare Wellness Visits

If you are covered by Medicare, you are eligible for a free Annual Medicare Wellness Visit to talk with your provider about ways to improve your health and prevent disease.  The Annual Wellness Visit is not the same as an annual physical exam, but it does provide patients with the same opportunity to talk with their provider about health concerns and to develop a plan in partnership with those providers to focus on health, rather than illness. 

Some of the services covered during an Annual Wellness Visit include:  routine measurements such as weight, height and blood pressure; review of your individual and family medical history, review of your current medications, discussion of the care you receive from other providers, and review of your functional ability and level of safety.  Based on this review, you and your provider will develop a health promotion and disease prevention plan based on your specific needs.

After your initial wellness visit, you are eligible for a follow-up or subsequent wellness visit every 12 months.  These visits are not subject to copayments to deductibles, so there is no cost to the individual. Collingsworth Family Medicine at 806-447-5311 to schedule your Annual Wellness Visit.

 

Narcotic Addiction help is available at Collingsworth Family Medicine

CFM follows evidence-based practices to help our patients overcome substance use disorders (SUD), such as addiction, dependency, and substance withdrawal.  We believe in a holistic approach by utilizing medication-assisted treatment (MAT) of FDA-approved medications combined with counseling and behavioral therapy to manage the symptoms of withdrawal.  Without the burden of these symptoms, patients can focus on positive behavioral and environmental changes to overcome their addictions or dependency.  

The benefits of MAT are well established in research to successfully treating opioid addiction and alcoholism.  Patients who actively participate in their own care with an open and honest relationship with their medical providers often achieve long-term recovery.   Professional counseling in combination with approved medications provides optimum recovery. 

CFM and CGH respect patients’ privacy and follow privacy laws.  All treatment is confidential and protected under the HIPAA Privacy Rule.

Collingsworth General Hospital and Collingsworth Family Medicine serve all patients regardless of ability to pay. 

Discounts for uninsured patients seeking medically necessary care provided by the hospital and clinic are offered based on the size of the patient’s immediate family, annual household income, and a sliding scale comparing household income with the hospital and clinic’s charity care income guidelines.  

Uninsured patients may apply for a discount in the hospital business office, or clinic front desk.

Providing your insurance information in advance will help speed you through check in at the time of your appointment.

Copays, coinsurance and deductibles are due at the time of service. Not all services, even those most people consider routine, will be covered by every insurance plan. If you have questions about specific benefits, please contact your insurance carrier. Services not covered by your insurance plan are the patient’s responsibility.

Bethany Pickard, MD, Internal Medicine

Bethany Pickard, MD

Bethany Pickard, MD, practices internal medicine at Collingsworth Family Medicine and Collingsworth General Hospital. She completed her Internal Medicine Residency at the Texas Tech University Health Sciences Center (TTUHSC), Amarillo, earned her Doctor of Medicine degree from TTUHSC School of Medicine, Lubbock, and her Bachelors of Science degree from Texas Tech University, Lubbock. She also attended the TTUHSC School of Pharmacy, Amarillo, for a year before deciding to go to medical school.

A second generation physician, Dr. Pickard was born in Clifton, Texas, and raised in Valley Mills, Texas. The family moved to Crosbyton, Texas, when Dr. Pickard was five, and her father continues to practice family medicine there. Dr. Pickard lives in Wellington with her husband Justin, an information technology professional and operations manager for Pickard Family Farms in Estacado, Texas.

Cassie Colson, RN, FNP

Cassie Colson, FNP

Cassie Colson, RN, FNP, received her Master of Science in Nursing – Family Nurse Practitioner degree from the University of Texas – Arlington in 2018, joining the providers at Collingsworth Family Medicine in January 2019. She started working as a nurse aide at Collingsworth General Hospital in 2010, while attending class at Amarillo College to earn her Associate Degree in nursing, and continued working as an RN while attending Oklahoma Panhandle State University where she earned her Bachelor’s Degree in Nursing.  

Cassie was born in Mississippi, but moved to Wellington when she was one year old, so she claims Wellington as her official hometown. She and her husband, Ryan, and two children, Rylyn and Caleb, live just west of the city limits of Wellington.

CASSIE COLSON, RN, FNP

Ryan Roberson, FNP

Ryan grew up in Wellington knowing that he wanted to help people and after a few medical mission trips after high school, he knew the medical field was right for him. Nursing seemed to be a calling, and after working as a Charge Nurse for an Intensive Care Unit in Amarillo, Ryan decided to take the steps needed to become a Family Nurse Practitioner.

Ryan lives near Wellington with his wife, Tana and his three children, years 5,7 and 9. They love the outdoors and spending time together hiking, biking and snow skiing.

Preston Barlow, PA

Preston comes to Wellington from Rocky Mountain University of Health Professions in Provo, Utah where he completed a Masters of Physician Assistant Studies. Preston is a National Registered Paramedic and also has a Bachelor of Science in Fire and Emergency Management. Preston is well-known to the surrounding communities within the Fire and EMS services and has served in public services for over 20 years as a firefighter, paramedic, and in law enforcement. He currently lives in Wellington with his wife and two children. With his family, he enjoys exploring dirt roads and seeing new places including a goal to visit all 50 states by his ten-year wedding anniversary. Preston’s other hobbies include fishing and working in his yard.

Preston Barlow, PA

Rajesh Nambiar, MD

Rajesh Nambiar, MD​

Rajesh Nambiar, MD, is a board certified interventional cardiologist who has been practicing with the Amarillo Heart Group for the last four years. He staffs a cardiology clinic at Collingsworth Family Medicine one day per month for the convenience of area patients. Dr. Nambiar completed his internal medicine training at Cook County Hospital, Chicago, Illinois, where he was also honored to serve as the chief resident. He furthered his training with fellowships in Cardiology and Interventional Cardiology at Cook County Hospital and Rush Medical Center, Chicago, respectively. As a result of this training, he is well equipped to manage cardiac disease and perform a wide range of diagnostic and interventional heart and vascular procedures.
